D Hospital Data Entry

Hello and welcome to this tutorial on D Hospital data entry.
Show learning Objectives Slide In this tutorial, we will learn to –
  • Complete the data entry of a new patient on the STEMI App at the time of direct admission to a D Hospital.
Show System Requirements Slide To practice this tutorial, you will need –
  1. An Android tablet with STEMI App installed on it and
  2. A working Internet connection

In – Thrombolysis

In a D Hospital data entry, the App will now take us to the next page which is Thrombolysis.
Under Thrombolysis first we have Medication Prior to Thrombolysis –

Based on Medications administered check the following

  • Asprin 325mg
  • Clopidogrel
  • Unfractionated Heparin
  • LMW Heparin

Ticagrelor

Under Thrombolysis, first we have Medication Prior to Thrombolysis.


Based on the medications administered to the patient in the hosptital, check Yes for the following-

  • Asprin 325mg If Yes, Dosage: 325mg Date and Time
  • Clopidogrel If Yes, Dosage, Date and Time
  • Unfractionated Heparin If Yes, Dosage, Date and Time
  • LMW Heparin If Yes, Dosage, Date and Time
  • Ticagrelor If Yes, Dosage, Date and Time
Only Narration Note that the Doses and selection of medications are an example meant for demo purposes.
Only Narration Administer drugs as per the patient's condition and treatment modalities
Select the Save & Continue button.

The App now take us to Thrombolysis. The App now take us to next page Thrombolysis.
Under Thrombolysis details

We have a mandatory field

  • Select any one type of thrombolytic Agent
  • Streptokinase
  • Tenecteplase
  • Reteplase


Once we select

  • Dosage
  • Start Date/ Time
  • End Date/ Time

After which we have the 90 MINS ECG DATE / TIME

and Successful Lysis Yes/ No.

Next, under Thrombolysis, we have a mandatory field.

* Select any one type of Thrombolytic Agent

  • Streptokinase
  • Tenecteplase
  • Reteplase

I'll select Streptokinase.


Once we select one of the Thrombolytic Agent, we have drop-downs

  • Dosage:
  • Start Date/ Time:
  • End Date/ Time:


After Thrombolysis, we have the

  • 90 MINS ECG DATE / TIME and
  • Successful Lysis Yes/ No

I will mark as YES.

This is based on the 90 mins ECG.

Make appropriate entries in these fields on your device.

Here, we have Medication in hospital
  • Nitroglycerine If YES, ROUTE:Oral Dosage:2.5mg Date and Time
  • Dopamine If YES ROUTE IV Dosage 5ml in 45ml of 0.9% ns Date and Time
  • Dobutamine If YES ROUTE IV Dosage 5ml in 45ml of 0.9% ns Date and Time
  • Adrenaline If YES ROUTE IV Dosage 4ml in 46ml of 0.9% ns Date and Time
  • Nor – Adrenaline If YES ROUTE IV Dosage 2ml in 48ml of 0.9% ns Date and Time
  • Other Drugs If YES, Name for example Vasopressin ROUTE IV Dosage 1ml in 19ml of 0.9% ns Date and Time
Note that the above doses and selection of medications are an example meant for demo purposes

Administer the drugs as per the patients's conditions and treatment modalities.

Under ADVERSE EVENTS page, we have to select either Yes or No.


On choosing Yes in each field, we will have to enter some additional information.

So, let’s start.

Adverse Reaction
  • Re - infarction if Yes
  • Location of Re-infarction: Inferior, Posterior, RV, Anterior, Lateral
    Date: Time:


  • Repeat PCI: Choose Yes and enter Date : Time:
  • CABG: Choose Yes and enter Date : Time:
  • Stroke Choose Yes and enter Date : Time:
  • Cardiogenic Shock Choose Yes and enter Date : Time:
  • Access Site Hemorrhage Choose Yes and enter Date : Time:
  • Major Bleed If Yes then enter Date : Time:
  • Minor Bleed If Yes then enter Date: Time:
  • Re-infarction: If Yes, then we have to enter the following-
  • Location of Re-infarction:
  • The options are Inferior, Posterior, RV, Anterior, Lateral
  • I will chooses Inferior and then enter the Date: Time:


  • Repeat PCI: If Yes then enter Date : and Time:
  • CABG: If Yes then enter Date : and Time:
  • Stroke IF Yes then enter Date : and Time:
  • Cardiogenic Shock If Yes then enter Date : and Time:
  • Access Site Hemorrhage If Yes and enter Date : and Time:
  • Major Bleed If Yes then enter Date : and Time:
  • Minor Bleed If Yes then enter Date: and Time:
Select all as 'NO Select ADVERSE EVENTS, if any and enter the Date and Time of its occurence.

For now I will select all as 'NO.

Discharge Summary Next, the App will take us to the next page Discharge Summary.
Discharge Summary >> Death


Mark as Yes


In the dropdown, select any one Reason for death

Cardiac, Non Cardiac, Others


Under Discharge Summary, we have Death.


If the patient dies, this is marked as Yes and we get a dropdown Reason for death. and Others.


Under reasons of death we have options- Cardiac, Non Cardiac.

If we choose either, we will get a drop down Death date and time.

Point to Others Choose the options Others to enter other reasons for the patient's death.

Then enter the Death date and time.

Once we choose any one options there ends the data entry.

Mark as ‘No’ and move on to Discharge / Transfer But if the patient did not die, then mark as No and move on to Discharge/Transfer.

I will select Death as No for now.

Select the Save & Continue button Select the Save & Continue button at the bottom of the page.

The App now takes us to the next page, that is Discharge Medications.
  • Aspirin: Yes

  • Beta Blocker: Yes
  • Nitrate
  • Statin: Yes
  • Others
Here we have

Mark a few of them as Yes The medications prescribed to the patient during discharge should be marked as Yes.

So, I’m going to mark a few of them as ‘Yes’.

Select the Save & Continue button Then select the Save & Continue button at the bottom.
Saved Successfully” message appears The page is saved and the success message will appear.

Discharge and transfer

The App will now proceed to take us to the next page, which is Discharge and Transfer.
Discharge from D Hospital - fill out the Date and Time.

In Discharge To field select one options

  • STEMI Cluster hospital
  • Non STEMI Cluster Hospital
  • Home
Discharge from D Hospital - fill out the Date and Time.

In Discharge To field select one options

  • STEMI Cluster Hospital
  • Non STEMI Cluster Hospital
  • Home
Select Home If we select Home, we get no further drop downs.
Select Non STEMI Cluster Hospital If Non STEMI Cluster Hospital then we have to manually enter these details.
  • Remarks
  • Transfer to Hospital Name
  • Transfer to Hospital Address

This is because details of Non STEMI hospitals are not available in the database.

Select STEMI Cluster Hospital
  • Remarks : If any
  • Transfer to Hospital name : Select Kovai Medical Centre and Hospital
  • Transfer to Hospital Address: 3209, Avinashi road, Sitra, Coimbatore, Tamil Nadu 641014
I am going to select STEMI Cluster Hospital.

If STEMI Cluster Hospital is selected, it further opens into Remarks field where we can add our remarks, if any.

Transfer to Hospital name : I will select Kovai Medical Centre and Hospital.

Transfer to Hospital Address: I will Select 3209, Avinashi road, Sitra, Coimbatore, Tamil Nadu 641014.

When we select the Hospital name the hospital address gets auto-populated.
Transport Vehicle : Choose Private Vehicle In Transport Vehicle field we have to choose between Private Vehicle and Ambulance.

I'm going to choose Private Vehicle.

Select the Save & Continue button

Based on the types of follow up, select from these options. I’ll select 1 mnth

Enter Follow up date Then, fill the Follow up date.
Mode of Follow – Hospital, Telephonic, Loss to follow up And then select the Mode of Follow upHospital, Telephonic, Lost to follow up

We have to select from this options based on the mode of follow up.

Select Loss to follow up If Lost to Follow up is selected we get no further drop downs.
Select Hospital or Telephonic

Type of Follow-up Hospital appears

STEMI, Non STEMI, No follow up


  • If STEMI is selected
    Name of Cluster &
    Name of Follow-up Hospital

shows up


  • If Non-STEMI is selected
    Name of Follow-up Hospital &
    Follow–up Hospital address

shows up

If Hospital or Telephonic mode is selected, we get the drop down - Type of Follow up Hospital .

Under Type of Follow up Hospital we get STEMI, Non STEMI, No follow up.

  • If No Follow up is done, then go to the next page by selecting the Save & Continue button at the bottom.
  • If Non-STEMI is selected, we have to enter the following details manually
    Name of Follow-up Hospital and
    Follow–up Hospital address

This is because this hospital is not part of the STEMI programme.


I am going to select STEMI.

  • If STEMI is selected we get drop downs.
  • Name of Cluster : I will choose KMCH
  • Name of Follow-up Hospital  : I will select Coimbatore medical college hospital
  • Follow up Hospital address: Trichy Road, Gopalapuram, Coimbatore, Tamil Nadu 641018, India

Notice that this address get auto populated. This is because this hospital is part of STEMI programme.

This page provides details of Events occurred till the time of follow-up.

Select ‘Yes’ to which of these events took place till the date of follow up.


I’ll check all as ‘No’ for now.

Click on the Finish button Lastly, select the Finish button at the bottom of the page.
