Title slide	Hello and welcome to this tutorial on transfer from STEMI D Hospital to STEMI A/B Hospital.
Opening Slide - can be graphic from the PPT	In this scenario, a patient is brought to a STEMI D Hospital first and is later transferred to a STEMI A/B Hospital.
Learning Objective Slide	In the STEMI D Hospital, the following details are to be entered into the STEMI App.
Blue box on already logged in D-User >> then on New Patient tab	Let’s begin. Note that in the STEMI Homepage, we are the D Hospital user. Select the New Patient Tab.
	Let’s assume a patient and enter the following details.
Enter Basic Details	Enter the patient’s name, age, gender, phone and address, as shown here.
Payment >> choose State BPL Insurance.	Next, choose the Payment from the drop-down. I’ll choose State BPL Insurance.
Enter the date and time of Symptom Onset.	Now, enter the date and time of Symptom Onset.
Blue box on STEMI D Hospital Arrival Date and Time.	Then, we are prompted to enter the STEMI D Hospital Arrival Date and Time.
Select Admission as Direct.	Next, select Admission as Direct. This is because the patient has directly presented to the STEMI D Hospital.
Blue box on STEMI Details.	After this, we have to fill out the STEMI Details.
Blue box on Manual ECG Taken	I’ll check Manual ECG Taken as Yes and then enter the Date and Time.
Blue box on STEMI Confirmed	Next, is STEMI Confirmed. This is confirmed based on the ECG taken. I’ll check STEMI Confirmed as Yes, along with Date and Time.
Blue box on Transport Details	Lastly on this page we have Transport Details.
Transport Details Here we will choose private vehicle in this case it could be Ambulance , GVK EMRI Ambulance or Public vehicle based on the mode of transport used	Here we will choose Private vehicle in this case. It could be Ambulance, GVK EMRI Ambulance or Public vehicle, based on the mode of transport used.
Save and Continue button.	Select the Save and Continue button at the bottom of the page.
Fibrinolytic Checklist.	The App will take us to the next page i.e. Fibrinolytic Checklist.
Check all as No.	Under Fibrionolytic Checklist, there are 12 items because the patient is Male. If the patient was Female, 13 items would be displayed. I will check No against each of the 12 items displayed on this page.
Save and Continue button.	After doing so, select the Save and Continue button at the bottom of the page.
Blue box on Cardiac History	We are now in the Cardiac History page.
Previous MI, Angina, CABG, PCI1, PCI2 >> No	I’ll check previous history Angina, CABG, PCI1, PCI2 as No.
Location of pain >> Retrosternal	Then under Diagnosis, enter the details for that particular patient.
Blue box on all items	After this, check the appropriate symptoms as observed in the patient.
Select a few as per narration	For symptoms I will check Palpitation, Diaphoresis, Shortness of breath, Nausea/vomiting, Dizziness as Yes.
Blue box on Clinical Examination	Next part is Clinical Examination.
Show filled details	Here we have to enter the Height, Weight and BMI of the patient.
Show filled details	And also BP Systolic, BP Diastolic and Heart Rate.
Save and Continue button.	Select the Save & Continue button at the bottom of the page.
Co-Morbid Conditions page	We are now taken to the next page - Co-Morbid Conditions.
	Enter the details after consulting with the patient or the patient’s family.
Show filled details.	I’m going to fill the following details.
Save and Continue button.	Select the Save & Continue button at the bottom of the page.
Contact Details page	We are now taken to the next page that is, Contact Details.
	Here we have to enter the details of the patient’s relative.
Show filled details	Name, Relation Type, Address, City, Contact No.
Show filled details	Then Occupation.
Show filled details	Then comes ID proof section - Aadhar Card No. and the upload of the soft-copy.
Save and Continue button.	Select Save & Continue button at the bottom.
Blue box on Thrombolysis	The App will now move us to the next tab which is Thrombolysis.
Blue box on Medications Prior to Thrombolysis.	Here we have to specify the Medications Prior to Thrombolysis.
Show filled details Blue box - narration	A list of medications is displayed on this page. Enter the details of the medication administered to the patient along with Dosage, Date and Time. Please note that the dosage of medications is an example meant for demo purposes. Administer drugs as per patient condition and treatment modalities.
Save and Continue button.	Select the Save & Continue button at the bottom.
Blue box on Thrombolysis	The App now takes us to the next tab which is Thrombolysis.
Blue box on Medications Prior to Thrombolysis.	Here we have to select any one Thrombolytic Agent as administrered.
Show filled details	I’ll select Streptokinase. Then enter the Dosage, Start Date and Time, End Date and Time.
Fill details of 90 minutes ECG.	This is followed by 90 minutes ECG, Date and Time.
Successful Lysis >> Yes	And Successful Lysis Yes/No which is determined based on the 90 mins ECG
Save and Continue button.	Select the Save & Continue button at the bottom.
Blue box on In- Hospital Summary	The App will now take us to the next page that is, In-Hospital Summary.
Under In hospital Summary >> Medication in Hospital	Here, we have Medication in hospital.
Mark a few of them as Yes	The medications administered to the patient in the STEMI D Hospital, should be marked as Yes.
Save and Continue button.	Select the Save & Continue button at the bottom.
Discharge Summary	We are now moved to Discharge Summary page.
	Under Discharge Summary, we have Death.
Select Death as No	I’ll select Death as ‘No’ for now.
Save and Continue button.	Then select the Save & Continue button at the bottom of the page.
Discharge Summary	The App will direct us to the next page, that is, Discharge Medications.
Mark a few of them as Yes	The medications prescribed to the patient during discharge, should be marked as Yes. So, I’ll mark a few as ‘Yes’.
Save and Continue button.	Then select the Save & Continue button at the bottom of the page.
Blue box on Discharge/Transfer.	The App will now take us to the next page, that is, Discharge/ Transfer.
Fill out Discharge from D hospital date and time	Discharge from D hospital. Fill out the Date and Time.
Discharge To >> STEMI Cluster Hospital	In *Discharge To field, select Stemi Cluster Hospital.
Select Transfer to Hospital Name:	Then select the Transfer to Hospital Name: from the drop-down list.
Point to address	On doing so, the Transfer to Hospital Address: gets auto-populated. This is because this hospital is already part of the STEMI programme.
Blue box on Transport Vehicle	In Transport Vehicle field, choose Private or GVK EMRI Ambulance. If Ambulance or GVK Ambulance is selected, we will be prompted to enter further details.
Click on Finish	Lastly, click on Finish button.
	This completes the data entry on the STEMI App in the STEMI D Hospital.
Slide	The patient has now arrived at an A/B Hospital.
Slide	This is the synopsis of the procedure to be followed at the A/B Hospital.
	Let's now learn how to enter further data on the STEMI App in the A/B Hospital.
Blue box on stemiAuser	Note that we are in the STEMI Homepage as an A/B Hospital user.
Select Search tab	Select the Search tab.
Type Venugopal >> select Search	Type the Id or name of the patient transferred from STEMI D Hospital. Then select the Search button at the bottom of the page.
	Now select the particular patient from the displayed list. Once the patient file is open, select the Edit button at the top right corner of the page. This will allow us to edit that particular page. Do likewise, to edit the remaining pages.
Blue box on Basic Details	We are now moved to the first tab Patient Details and the first page– Basic Details.
	Here, all the details filled in the STEMI D Hospital are displayed.
Blue box on A/B Hospital Arrival Date and Time.	In the A/B Hospital, we will have to fill the A/B Hospital Arrival Date and Time.
Select Save and Continue	Select the Save and Continue button at the bottom.
Blue box on Fibrinolytic Checklist	We can skip this page as data was already entered here at the STEMI D Hospital.
	The Thrombolysis main tab is skipped. Reason is - Thrombolysis was done at the STEMI D Hospital along with its data entry.
Blue box on PCI	The App now takes us to the next tab, PCI.
Blue box on Drugs before PCI	Here we have Drugs before PCI page.
Make entries in PCI	Enter the details of the drugs administered to the patient before PCI, with Date and Time details.
Select Save and Continue	Select the Save and Continue button at the bottom.
On PCI page	The next page is PCI Details.
Cath Lab Activation and Cath Lab Arrival	Data under PCI details should be filled either by the cardiologist or the Cath Lab Technician. Let’s start the data entry in this page by entering Cath Lab Activation and Cath Lab Arrival.
Blue box on Vascular access Blue box on Catheter access.	Next we have Vascular access followed by Catheter access.
Blue box on CART, Start & End Date and Time	Then fill the CART details such as Start Date and Time End Date and Time
Blue box on Culprit Vessel	After this, we have to specify any one Culprit Vessel from the options provided.
	Then enter all the details relevant for that Culprit Vessel.
Blue box on Management	Now, make the relevant entries under Management for that patient.
Blue box on Intervention	Lastly on this page, we have Intervention.
Select Intervention	When the option Intervention is selected, we get some more details below.
	Fill out the ones that are relevant for that particular patient.
Select Save and Continue	Select the Save and Continue button at the bottom.
Blue box on Medications in Cath Lab	We are in the Medications in Cath Lab page.
	Enter the details of the medications administered to the patient in the Cath Lab.
Blue box narration	I will enter the details for 2b3a Inhibitors. Unfractionated Heparin Dosage, Date and Time. Please note that the above dosage and selection of medications is an example meant for demo purposes. Advise drugs as per patient’s condition and treatment modalities.
Select Save and Continue	Then select the Save and Continue button at the bottom.
	We will skip the In-Hospital Summary page
Blue box on Discharge Summary >> Death	And move on to the next page that is Discharge Summary. Here we have the tab Death.
Select No.	I will select the option No.
Select Save and Continue	Then select the Save and Continue button at the bottom.
Blue box on Discharge Medications.	Next comes Discharge Medications.
	Once again, enter the relevant ones for that patient, from the options displayed on this page.
Select Save and Continue	Then select the Save and Continue button at the bottom.
Blue box on Discharge/Transfer	Now we are taken to the Discharge or Transfer page. Here are the discharge details of the patient. These were entered earlier, when the patient was transferred from STEMI D to STEMI AB Hospital.
Select Add Transfer Details	Here, select the Add Transfer Details button.
Blue box on Discharge from the A/B Hospital	Now, we need to enter the details related to the discharge from the A/B Hospital.
Select Finish	Lastly, select the Finish button.
	Let us summarize.
Show Summary Slide	In this tutorial, we have learnt to - Enter data of a new patient on the STEMI App first in the STEMI D Hospital And enter further data for the same patient on the STEMI App in an A/B Hospital.
