VISUAL CUE NARRATION
Title slide Hello and welcome to this tutorial on Transfer from Non-STEMI C Hospital to STEMI D Hospital.
Opening Slide - can be graphic from the PPT In this scenario, a patient is brought from a Non STEMI C Hospital to a STEMI D Hospital.
Learning Objective Slide In the STEMI D Hospital, the following details are to be entered into the STEMI App.
Blue box on already logged in D-User >> then on New Patient tab Let’s begin.

Note that in the STEMI Homepage, we are a D Hospital user.

Select the New Patient Tab.

Let’s assume a patient and enter the following details.
Enter Basic Details Enter the patient’s name, age, gender, phone and address, as shown here.
Payment >>choose State BPL Insurance. Next, choose the Payment from the drop-down.

I’ll choose State BPL Insurance.

Now, enter the date and time of Symptom Onset.
D Hospital Arrival Date and Time Then, we are prompted to enter the STEMI D Hospital Arrival Date and Time.

Next, select Admission as Non-STEMI.

Recall - this is a transfer from Non STEMI C Hospital to STEMI D Hospital.

We need to enter all the details in this case.

No data is transferred from Non STEMI Hospital to the STEMI Hospital.

This is because Non STEMI Hospitals are outside the STEMI programme.

The data flow only takes place between the hospitals linked within the STEMI programme.

Enter details Next, enter the hospital name and address.
Blue box on Non STEMI Hospital Arrival Date and Time. Then, we are prompted to enter the Non STEMI C Hospital Arrival Date and Time.
After this, we have to fill out the STEMI Details.
Blue box on Manual ECG Taken I’ll check Manual ECG Taken as Yes and then enter the date and time.
Blue box on STEMI Confirmed Next, I’ll check STEMI Confirmed as Yes, along with date and time.

I’ll check Post Thrombolysis as No as the patient was transfered from a C type Non STEMI Hospital.

In these hospitals, Thrombolysis is not done.

Lastly on this page, we have Transport Details.
Transport Details. Here we will choose Private or GVK EMRI Ambulance.
Save and Continue button. Select the Save and Continue button at the bottom of the page.
Fibrinolytic Checklist. The App will take us to the next page i.e. Fibrinolytic Checklist.

Check all as No.

Under Fibrinolytic Checklist, there are 12 items because the patient is Male.

If the patient was Female, 13 items would be displayed.

I will check No against each of the 12 items displayed on this page.

Save and Continue button. After doing so, select the Save and Continue button at the bottom of the page.
Cardiac History page We are now in the Cardiac History page.
Previous MI, Angina, CABG, PCI1, PCI2 I’ll check previous history Angina, CABG, PCI1, PCI2 as No.
Chest discomfort - Pain

Location of pain - Retrosternal.

Pain Severity - 8.

Then under Diagnosis, I’ll enter the following details-

Location of pain - Retrosternal.

Pain Severity - 8.

Blue box on all items After this, check the appropriate symptoms as observed in the patient.
Select a few as per narration For symptoms, I will check

Palpitation, Diaphoresis, Shortness of breath, Nausea/vomiting, Dizziness

as Yes.

Blue box on Clinical Examination Next part is Clinical Examination.
Show filled details Here we have to enter the Height, Weight and BMI of the patient.
Show filled details And also BP Systolic, BP Diastolic and Heart Rate.
Save and Continue button. Select the Save & Continue button at the bottom of the page.
Co-Morbid Conditions page We are now taken to the next page - Co-Morbid Conditions.
Enter the details after consulting with the patient or the patient’s family.
Show filled details. I’m going to fill the following details.
Save and Continue button. Select the Save & Continue button at the bottom of the page.
Contact Details page We are now taken to the next page that is Contact Details.
Here we have to enter the details of the patient’s relative.
Show filled details Name, Relation Type, Address, City, Contact No.
Show filled details Then Occupation.
Show filled details Then comes ID proof section - Aadhar Card No. and the upload of the soft-copy.
Save and Continue button. Select Save & Continue button at the bottom.
Blue box on Thrombolysis The App will now move us to the next tab which is Thrombolysis.
Blue box on Medications Prior to Thrombolysis. Here we have to specify the Medications Prior to Thrombolysis.

A list of medications is displayed on this page.

Enter the details of the medication administered to the patient along with dosage, date and time.

Save and Continue button. Select Save & Continue button at the bottom.
Blue box on Thrombolysis The App will now move us to the next page that is Thrombolysis.
Select Streptokinase

Here we have to select any one type of Thrombolytic Agent.

I’ll select Streptokinase.

Then enter the Dosage, Start Date and Time, End Date and Time.

Show filled details This is followed by 90 min ECG, Date and Time.

And Successful Lysis Yes or No.

This is based on the 90 mins ECG.

Save and Continue button. Select the Save & Continue button at the bottom.
Blue box on In-Hospital Summary. We’ll skip the next tab, that is In-Hospital Summary page.
Discharge Summary We are now in the Discharge Summary page.
Blue box on Death >> No. Under Discharge Summary, we have Death.


I’ll select Death as ‘No’ for now.

Save and Continue button. Then select the Save & Continue button at the bottom of the page.
Blue box on Discharge Medications. The App will direct us to the next page, that is, Discharge Medications.
Select few as Yes

The medications prescribed to the patient during discharge should be marked as Yes.


So I’ll mark a few as ‘Yes’.

Save and Continue button. Then select the Save & Continue button at the bottom of the page.
Next page Discharge/ Transfer The App will now take us to the next page that is Discharge/ Transfer.
Discharge from D hospital

Date: 21/1/2016 Time: 9:45 am

Discharge from D hospital

Fill out the Date & Time.

Discharge To >> select Stemi Cluster Hospital In Discharge To field, select Stemi Cluster Hospital.
Select Transfer to Hospital Name >> Kovai Medical Then select the Transfer to Hospital Name from the drop-down list.

On doing so, the Transfer Hospital Address gets auto-populated.

This is because this hospital is part of the STEMI programme.

Blue box on Transport Vehicle. In Transport Vehicle field, choose Private or GVK EMRI Ambulance.
Click on Finish Lastly, click on Finish button.
This completes the data-entry when a patient is transferred
  • from a Non-STEMI C Hospital
  • to a STEMI D Hospital.
