C Hospital Data Entry

Hello (or any preferred greeting) and welcome to this tutorial on C Hospital data entry.
Show Learning Objectives Slide In this tutorial, we will learn to –
  • Complete the data entry of a new patient on the STEMI App at the time of direct admission to a C Hospital.


Show System Requirements Slide To practice this tutorial, you will need –
  1. An Android tablet with STEMI App installed on it and
  2. A working Internet connection

  • You will also need to have working knowledge of the STEMI device and
the STEMI App.

In – Hospital Summary In the C Hospital Data Entry, the App will take us to the next page which is In – Hospital Summary.
Under In hospital Summary we have Medication in Hospital
  • Nitroglycerine if Yes

Route: Oral Dosage: 2.5 mg Date: 21/1/2016 Time: 9:30 am


  • Dopamine if Yes

Route: IV Dosage: 5ml in 45ml of 0.9% NS Date: 21/1/2016 Time: 9:32 am


  • Dobutamine if Yes

  • Cardiogenic Shock if Yes, then enter Date: and Time:
  • Access Site Hemorrhage: if Yes, then enter Date: and Time:

For now I’ll select all as’No


Select Adverse Events from the above if any, and enter Date: and Time: of its occurence.


For now I’ll select all as ’No’.

Select the Save & Continue button at the bottom of the page Then select the Save & Continue button at the bottom of the page.

And the success message will appear at the bottom.

Discharge Summary The App will now take us to the next page, that is Discharge Summary.
Under Discharge Summary we have Death Under Discharge Summary, we have Death.
Mark as Yes

Highlight the dropdowns

*Reason for death

*Others

If the patient dies, this is marked as “Yes” and we get a dropdown - Reason of death and Others.
Reason for death >>Cardiac >> Non Cardiac Under Reason of death

we have options- Cardiac and Non Cardiac.

Highlight Death Date and Time:


If we choose either, we get a drop-down Death Date and Time:
Highlight Others >> enter the Death Date and Time.


In Others: we have to enter other reasons for the patient’s death.


Then enter the Death Date and Time.

End of data entry

Highlight Death Bar on top and Save and Continue button in Blue box at the same time.

Once we choose any one option, there ends the data entry.
if not mark as ‘No’ and move on to Discharge Medications



I’ll select Death as ‘No’ for now

If the patient did not die, then mark as “No” and move on to Discharge Medications.

The details of Non-STEMI Hospital are not available in the database.

Select Stemi Cluster Hospital. I’m going to select Stemi Cluster Hospital.




Highlight Remarks field, Transfer to Hospital Name:, Transfer to Hospital Address:

On selecting STEMI Cluster Hospital, it further opens into-
  • Remarks field, where we can add our remarks, if any.
  • Then Transfer to Hospital Name: I will put Kovai Medical Center and Hospital
  • Transfer to Hospital Address: 3209, Avinashi Road, Sitra,

Coimbatore, Tamil Nadu - 641 014

Blue text box on screen When we select the hospital name, the hospital address gets auto-populated.


This is because this hospital is already part of the STEMI programme.

Transport Vehicle Choose from -
  • Private
  • Ambulance

I’ll select Private

In Transport Vehicle field, we can choose
  • Private Vehicle
  • or Ambulance

I’m going to choose Private Vehicle.

Select the Save & Continue button at the bottom of the page Select the Save & Continue button at the bottom.

Select Telephonic If Hospital/ Telephonic mode is selected, we get the drop-down Type of follow-up Hospital.

Select Non STEMI >> enter the Name of the Follow- Up Hospital and Follow- Up Hospital Address If ‘Non STEMI’ is selected, we will have to enter the following details manually-
  • Name of the Follow- Up Hospital
  • Follow- Up Hospital Address
